Smaller incisions, muscle-sparing approach, quick recovery, and 30% cost savings.
Dr. Thana Turajane, M.D. of Police General Hospital, a skillful surgeon specializing in Total Hip Arthroplasty (THA) surgery, recently gave a special lecture on an innovative technique of “Cementless Minimally Invasive Anterolateral Total Hip Arthroplasty (Cementless MIS AL THA)” at the Orthopaedics room, Police General Hospital. The purpose was to make known the availability and benefits of this new technique to general public, especially the hip osteoporosis patients.
The hip osteoporosis patients in a serious condition suffer a painful and swollen hip that can limit their movements and effect their daily activities. Normally, the physicians will analyze the disorder by looking at the x-ray film and the hip check-up report, which can indicate clearly where the fracture or the deformity of the hipbone located. If taking the medicines and/or physical therapy are not working, the surgeons will advise patients to have the THA surgery. In the past, the incision size for the standard hip replacement was large and about 20-30 centimeter long and some muscles would be cut and lost in the process. Then the patients needed to stay in the hospital for another 2 weeks after the surgery for recovery.
With the fast development of the technology for orthopaedic procedure and hip replacement, the orthopaedic surgeons at the Police General Hospital can now offer the new surgical procedure called “Cementless MIS AL THA”. By using this new procedure, the incision size will be much smaller avoiding unnecessary muscle cut, lesser invasive, quicker recovery, shorter hospital stay and also the patients can save the cost by 30% when compared to standard surgery.
Today’s standard hip replacement, the physicians need to open the incision 20-30 centimeter wide and there are two kinds of hip prosthesis: with or without bone cement. However, MIS AL THA are now starting to gain popularity amongst the orthopaedic surgeons worldwide as the average incision is much smaller (up to 50%) avoiding the lost of muscles around the hip. This new procedure goes along well with the cementless prosthesis since the physicians do not need to concern about the bone cement but rather leave it to the nature of human body that can recreate the bone around the hip prosthesis automatically. Moreover, the cementless THA requires less operation time, resulting in less blood loosing, and reducing the risk of infection. In addition, the physicians also found that the patients who gone through cementless THA can keep the quality of the post-operation bone stock as close to or even similar to the pre-operation bone stock. This helps make hip prosthesis stronger and more endurable. That explains why the cementless THA becomes the best choice for young patients whose pre-operation bone stock is not too thin.
The multi-benefit Cementless MIS AL THA brings better quality of life to the patients. They are only required to stay in the hospital for one week and can go back home and continue with their daily activities. The total costs for the procedure are 120,000 baht which is 30 percent saver when comparing to the standard procedure due to the shorter hospital stay and the less room, medicine and nursing cost.

I think this is the next step in our approach to make minimally invasive joint replacement surgery accessible to a greater number of surgeons and patients because this approach has been developed on the consideration of patients’ benefits and their quality of life,” said Dr. Thana Turajane, M.D. of Police General Hospital.
Cementless THA consists of 4 parts: Femoral Stem, Femoral Head, Acetabular Cup and Acetabular Cup Liner, which is a special, light, safe and durable plastic well-known to orthopaedic surgeons worldwide with over 400,000 patients worldwide using this prosthesis (1983-2005). Ninety eight point six percent of all patients can use this prosthesis longer than 10 years, affirmed by many studies conducted all over the world. For example:
1. Journal: AAOS-Eighteen years follow-up of the CLS uncemted stem, 2004 by Grappiolo/Spotorna, 98 percent of 300 patients can use the prosthesis for 16 years

In addition, there are over 100 published reports saying the result of using Femoral Stem from the studies of the worldwide leading medical institutions.
Ms. Puntarik Ounhaka, 35, a patient who had gone through Cementless MIS AL THA gave the interview “I have SLE (Systemic Lupus Erythematosus), then I found myself having a hip problem. From the x-ray film, the doctor said my hipbone was deformed making my legs unequally so I need to get surgery treatment. Only 2 days after the surgery, the doctor let me try to walk by using the helping tool. Then, I felt better and felt less pain and I continued to walk better. Only one week, the doctor allowed me to go back home and my scar is only 10-12 centimeters and now I have a better quality of life and can drive normally”.
At present, Police General Hospital is using this new procedure: Cementless MIS AL THA. The physicians found that most patients can walk within 1-2 days after the surgery and can continue with their normal daily activities within 3 weeks. This new procedure causes less issue bruises, faster rehabilitation, lesser pain, smaller incisions, less scarring, and shorter hospital stay. So, this new procedure can offer potential benefits and value-added to the quality of life for patients.
